Vegan Primary Care has closed, but your records are safely kept and available to you. Use this page to request a copy of your own records, or to have them sent to another doctor.
How it works
- 1Fill out a short formAbout 3 minutes. You'll need your date of birth and the email or phone number you used with our office.
- 2We confirm it's youWe check your request against our records. If anything has changed, we'll ask for a photo ID.
- 3Get a secure download linkWe email a private link to the address we have on file. The link works for 7 days.
Good to know
- There is no charge for a copy of your records.
- Every request is answered within 30 days, usually much sooner.
- Requesting for someone else, or from another doctor's office? Use the same form. A signed authorization from the patient is required. Download the authorization form (PDF)
